With the recent focus on reducing hospital readmissions, most of us are familiar with the stats: one in five Medicare patients is readmitted within 30 days, costing the health care system $26 billion each year.
But the numbers don’t tell patients’ stories. Avoidance, fear and confusion can impact a patient’s care at home and result in another trip to the hospital.
